A Subtle, Sculpted Refresh at 64: Full Face Rejuvenation Case Study

When Jackie, aged 64, visited me at my North Manchester clinic within Jems in Whitefield, she was looking for a subtle “spruce up” ahead of her son’s upcoming wedding. Her goal wasn’t to look different, but to feel fresher, lighter, and more confident in her appearance.
Patient Background & Concerns
Jackie had previously undergone dermal filler treatment to her marionette lines in 2016 but hadn’t had any injectable treatments since. Her main concern was the lower face, specifically the lines around her mouth and lower face that had developed over time. Consultation & Assessment: A Skin-to-Bone Approach
When treating concerns related to ageing, I always take a skin-to-bone approach, assessing every layer of the face to understand why changes have occurred, not just where they are visible.
In Jackie’s case, the key findings were:
Volume loss in both deep and superficial fat pads
Structural changes to the underlying bone
Resulting heaviness and shadowing in the lower face
These findings are really important to consider when implementing a treatment plan, because directly treating lines (such as marionette lines) without addressing foundational volume loss can actually make the lower face appear heavier, the opposite of what we want to achieve.

After a thorough consultation, we decided on a full face rejuvenation using dermal filler to restore structure, support, and lift.
Jackie was treated over two sessions, using a total of 7ml of dermal filler across the following areas:
Cheeks (to restore mid-face support)
Chin (to improve projection and balance)
Jawline (to define and reduce heaviness)
Pyriform fossa (to support the mid-face and soften folds)
Pre-jowl area (to smooth transitions along the jawline)
Pre-auricular area (to improve overall facial contour)
By restoring volume in these key structural areas, we were able to indirectly improve the appearance of the lower face, rather than simply “filling lines”.
